29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached: On or about July 9, 2019, employees were exposed to fall hazards 12.1 feet to the ground below when performing roofing activities on a low sloped roof and three employees were attached to the same anchor point which would not support at least 15,000 pounds.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could neither free fall more than 6 feet (1.8 m), nor contact any lower level: On or about July 9, 2019, employees were exposed to fall hazards 12.1 feet to the ground below when performing roofing activities on a low sloped roof and the rope grabs were not positioned to prevent employees from contacting the ground below.
