Site: 1463 61st Street, Brooklyn, NY 11219 Location: a) On the North-East, back wall of the excavation, on the worksite On or about 07/10/2019, employees were cleaning up debris on the jobsite and were exposed to falling approximately 7 feet to the lower level. Employees were standing on the back foundation wall of the excavation without the use of guardrails, safety nets, or personal fall arrest systems when working near the exposed edge.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Site: 1463 61st Street, Brooklyn, NY 11219 Location: a) Towards the South-West, front entrance of the excavation, on the worksite. On or about 07/10/2019, employees used a folded 12 foot A-frame ladder for access to and from the excavation. The ladders side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed.
