29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On the east side of the construction site, employees performing masonry activities were not provided a scaffold ladder to access and egress the two tier tubular welded frame scaffold.  Employees were expose to potential fall hazards.   WCS Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 1205552, citation number 01, item number 001 and was affirmed as a final order on 03/17/2017, with respect to a workplace located at 6706 Westworth Blvd., Westworth Village, Texas 76114.  WCS Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 985872, citation number 01, item number 003 and was affirmed as a final order on 10/17/2014, with respect to a workplace located at 1320 Loop 288, Denton, Texas 76205.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   On the east side of the construction site, employees performing masonry activities on a commercial building worked from a tubular welded frame scaffold system without fall protection.  The employees were expose to a fall hazard of 12 feet to the ground below.   WCS Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1205552, citation number 01, item number 002 and was affirmed as a final order on 03/17/2017, with respect to a workplace located at 6706 Westworth Blvd., Westworth Village, Texas 76114.
