OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee exposed to fall hazards while working from powered industrial trucks:  Location: 373 Rt 46 W Bldg E Fairfield, NJ 07004-warehouse   a) On or about 07/10/2019 an employee was exposed to fall hazards of approximately 5 feet while standing on a pallet and being raised by a forklift in order to repair damaged sheet rock near the ceiling.  The worker was not provided with and did not utilize fall protection during the operation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Location: 373 Rt 46 W Bldg E Fairfield, NJ 07004-Sampling Department  a) Employees were exposed to crushing and amputation injuries while mixing and pouring paint/color solutions on unguarded rotating rollers of the Chint rolling machine QC36-30T on or about 7/11/19.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:.  Location: 373 Rt 46 W Bldg E Fairfield, NJ 07004-Sampling Department  a) Employees were exposed to electrical shock and fire while using light duty relocatable power taps as a permanent connection to power/charge various rollers, ventilation and scale devices.  The power taps were not used as per their listing and labelling thereby increasing overheating and posing a fire hazard on or about 7/11/19.
