The step ladders were in the closed positions and leaning upright to the building. The ladders were not designed to be used in this fashion. This occurred on or about 07/10/19.

29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extended at least 3 feet (0.9 m) above the upper landing surface a) On 0710/19, At site of Mabee Fieldhouse, 11901 Warnell Rd, KC MO -- employee did not have portable ladder (extension ladder) that extended 3 feet above upper landing surface for access. This exposed the employee to a fall hazard of more than 13 feet, 8 feet and 10 feet at three different locations. These falls could result in death or serious trauma.
