If no entry is performed during a 12-month period, no review is necessary.   Armour Industrial Services, Inc, Athens, GA - An employee entered an E.Q. basin (waste pit) at Pilgrim's Pride, classified by the company as a permit required confined space. The written permit required confined space (PRCS) was not reviewed and updated to comply with the standard. The written program stated that:  a) The OSHA's confined space standard, 1910.146 only applies to construction contractors at industrial facilities; b) The entry permits were not being signed by the client's entry supervisor as stated in the program; c) The procedures stated that the client's procedures should contained the procedures to summon and prevent unauthorized personnel from attempting any rescue but the clients procedures were not obtained and reviewed for compliance prior to entry, on or about 7/9/19
