29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. a) The employer did not ensure that the (flat) roof will support employees and equipment during roof repair that required removal of part of the deteriorated concrete of the roof slab; employee and the machine he was on broke through the roof and fell 15 feet to the 4th floor. Location: 6500 16th Ave, Brooklyn, NY; School I.S. 227 On or about 7/5/2019 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. a) When removing deteriorated portions of a concrete roof slab that was weakened by water damage the employer did not do continuing inspections to determine if the roof can support the load of equipment and employees. Employees were exposed to structural collapse hazard. Location: 6500 16th Ave, Brooklyn, NY; School I.S. 227 On or about 7/5/2019 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
