Integral ladders in fabricated frames shall: 29 CFR 1926.451(e)(6)(i): Be specifically designed and constructed for use as ladder rungs; On July 10, 2019, employees were exposed to falls while climbing fabricated frame scaffolding that was not intended to be used as a ladder since the rung spacing and width did not meet the requirements of a ladder.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. On and about July 10, 2019, employees were exposed to falls from scaffolding that was not equipped with complete guardrails along the length of working levels or any guardrail on the ends of work brackets higher than 10' above the ground.
