29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about 10 July, 2019, in the front of the residential building, two employees worked from an aerial lift, one of which did not utilize fall protection to prevent him from falling out or being ejected from the basket.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: On or about 10 July, 2019, on the front side of the building, an employee working from an aerial lift accessed a balcony by climbing over the balcony rail with a break in elevation over 19 inches without the use of a ladder, exposing the employee to a fall hazard of approximately 15 feet.
