On or about July 5, 2019; at the new construction site where the employer did not ensure that authorized employees who perform maintenance work on equipment received training on the hazardous energy sources of that equipment.

29 CFR 1910.147(f)(3): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work: a) This violation occurred on or about July 5, 2019; at the new construction site, where employees who were performing maintenance on the Rapid Mix 400 CW were not provided with individual locks and/or tags to ensure equipment to ensure was not re-energized prior to work being completed.
