29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) On or about 7/11/2019 - 6219 Tattersall Blvd in Hoover, AL, employees were installing 12 inch water pipe and 36 inch storm drain pipe in an excavation approximately 34 feet long, 20 feet wide and 8 feet deep without proper access.

29 CFR 1926.651(j)(2): Employees shall be protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: (a) On or about 07/11/2019 - 6219 Tattersall Blvd.in Hoover, AL, employees were exposed to struck-by hazards while installing 12 inch water pipe and 36 inch storm drain pipe in an excavation.
