29 CFR 1926.20(b)(2):  Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer.   Quality Seamless Guttering did not conduct an adequate inspection of the job site, materials and equipment. During the inspection, employees were exposed to the following hazards:  a)  One employee was exposed to a fall hazard while working on an upper landing of a residential building. b)  One employee was exposed to hazards associated with falling objects while working directly below a coworker. c)  Employees were using a ladder to access the upper portion of a porch of a residential building. The ladder was not extended at least 36 inches above the upper landing's edge.

29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On June 26, 2019, an employee working for Quality Seamless Guttering at 5571 Wells Ave, St. Louis, MO was not wearing a protective helmet while working beneath his coworker on the roof. The employee was subject to head injuries from possible falling objects and tools.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  On 06/26/2019, an employee of Quality Seamless Guttering Inc. was working on the roof of a residential building located at 5571 Wells Ave in St Louis, MO. The exposed employee was not utilizing a guardrail system, safety net system, or a personal fall arrest system. The employee was observed on the roof of the building without fall protection, exposing him to a fall to the ground below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  On June 26, 2019, employees of Quality Seamless Guttering were working on an upper landing of a residential building at 5571 Wells Ave, St. Louis, Missouri. These employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate hazards.

29 CFR 1926.1053(b)(1):  When a ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 meters) above the upper landing surface to which the ladder is used to gain access:  On June 26th, 2019, workers from Quality Seamless Guttering were on the upper landing at a residential building located at 5571 Wells Ave, St Louis, Missouri, were using a ladder improperly. The ladder was not extended three feet beyond the landing or edge of the roof.
