29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: 2ND FLOOR, 1 BRIGHT MEADOW BOULEVARD, ENFIELD, CT: On or about and at times prior to July 11, 2019, the employer did not ensure that all electrical equipment at the site was being used in accordance with the listed use. In particular, A) A relocatable power tap (RPT) was in use in the 2nd floor hallway. Five of the six outlets were being used to energize an air conditioner, a circular saw, floor lamps, and a shop vacuum. B) An Insignia portable air conditioner was in use in the Board Room. C) A metal outlet quad box was being used as a relocatable power tap.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: 2ND FLOOR, 1 BRIGHT MEADOW BOULEVARD, ENFIELD, CT: On or about and at times prior to, flexible cords that were extended across hallways and through doorways were covered with duct tape to mitigate tripping hazards. The duct tape did not protect the cords form damage as they were walked on and/or equipment was rolled over them.
