29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: On or about July 1, 2019, at the job site employees were observed scaling on the scaffolding frames as a means to access the scaffolds, no ladder was provided. Employees were exposed fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about July 10, 2019, at the jobsite employees were observed working on the third level of a 3-tier scaffold approximately 18 feet high. The scaffold did not have guardrails provided and the employees were not wearing personal fall arrest systems. Employees were exposed to fall hazards. Urbano Gonzalez Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1394690, citation number 1, item number 1, and was affirmed as a final order on June 3, 2019 with a final abatement date of April 18, 2019, with respect to a workplace hazard at 400 NE 50th St., Oklahoma City, OK 73015.
