29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person  Location: 33 Madaket Rd. Nantucket, MA 02554  On July 11th, 2019, employees were exposed to falls hazards, struck by hazards, and electrical shock hazards while working on a residential construction project where frequent and regular inspections were not conducted by a competent person to identify and mitigate the hazards present during the inspection:   Instances:  a) Employees were exposed to falls from heights up to 10 feet to the ground while installing cedar shingles and painting the turret where fall protection was not utilized. b) Employees were exposed to falls from heights up to 10 feet to the ground while accessing the roof where the portable ladder was not extended at least three feet above the upper landing surface. c) Employees were exposed to falls from heights up to 8 feet to the ground while walking where door openings were not guarded. d) Employees were exposed to falls to a lower level while walking where handrails were not installed on stairs ways.  e) Employees were exposed to falls over two feet while walking where stairs or ladders were not provided at all access ways with a break in elevation of over 24 inches was present.  f) Employees were exposed to struck-by, overhead, falling object hazards while walking and carpenters were working above the entryway and around the exterior of the building where head protection was not utilized throughout the project. g) Employees were exposed to electrical shock hazards while working where power tools were energized with damaged electrical extension cords. h) Employees were exposed to struck-by hazards while working where table saw guards were not utilized or present. I) Employees were exposed to caught-between hazards while installing insulation around the concrete foundation in a four and a half foot deep excavation where a trenching and excavation competent person did not inspect and ensure the excavation was safe before the workers entered the excavated area.
