On or about July 10, 2019, in the production area, employees were exposed to smoke and fire hazards when the exit access on various production lines was less than 28 inches wide. The exit access between lines 5 and 6 was approximately thirteen (13) wide.

On or about July 10, 2019, in the facility, employees were exposed to machine hazards associated with rotating and moving parts when they engaged in servicing and maintenance activities on equipment located in production. The employer did not ensure that employees implemented energy control application steps prior to allowing them to perform sanitation and change overs, exposing them to crushing and amputation hazards. b. On or about July 10, 2019, in the sanitation department, an employee(s) was exposed to amputation hazards when sanitizing line 32. The employer did not ensure that sanitizer employee(s) implemented energy control applications steps prior to accessing moving equipment in the filling line after machine guarding has been removed.
