29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) Facility    An employee working as a picker was exposed to continuous noise levels at 60.5 %  of the Permissible Exposure Limit (8 hour Time Weighted Average sound level of 90 dBA) which is 0.605  times the allowable exposure limit.   The action level for employees who work 10.5 hours,  is  83.0 dBA.  The equivalent sound level of the measured dose is 86.3 dBA.  Sampling was performed for 468  minutes on 09/17/19, with the unsampled portion of time considered to represent zero for the exposure calculation.    An employee working as a cleaner was exposed to continuous noise levels at 69.8 %  of the Permissible Exposure Limit (8 hour Time Weighted Average sound level of 90 dBA) which is 0.698  times the allowable exposure limit.   The action level for employees who work 10.5 hours,  is  83.0 dBA.  The equivalent sound level of the measured dose is 87.4 dBA.  Sampling was performed for 463  minutes on 09/17/19, with the unsampled portion of time considered to represent zero for the exposure calculation.    Violation occurred on or about 09/17/19.
