29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about July 13, 2019, the employer did not ensure fall protection was implemented for each employee during commercial construction operations when workers were exposed to the hazard of falling approximately twenty-four (24) feet when they were installing roof singles.  a. Three employees were not provided with a means of fall protection. b. Workers who were provided body belts and ropes were not protected with suitably installed anchorage.

29 CFR 1926.1053(b)(1): Portable ladders are used for access to an upper landing surface and the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    On or about July 13, 2019, the employer did not ensure a portable ladder was constructed and placed so three (3) feet or more was extended above the upper landing.  Workers performing commercial roofing were exposed to falling approximately ten (10) feet while using an extension ladder to ascend/descend from the steep pitch roof to the porch roof.
