OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that a maintenance employee was exposed to the hazard of being struck-by/crushed by. A) 475 Exterior Street, Bronx NY: In the maintenance garage On or about 7/13/19, a maintenance employee was crushed to death while welding stress cracks, seams, and reinforcing plates on an excavation bucket that had been removed from a Komatsu PC-490.
