29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a- Roof Exterior: An employee engaged in solar panel installation activities on a 4 on 12 pitched roof was exposed to fall hazards of approximately 20ft to the concrete below as a means of fall protection was not utilized. Condition noted on or about 07/15/2019.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a- Roof Exterior:  An employee engaged in solar panel activities on a 4 on 12 pitched roof was exposed to fall hazards of approximately 20ft to the concrete below as an extension ladder used for access whose side rails did not extend 3ft above the upper landing surface. Condition noted on or about 07/15/2019.
