29 CFR 1926.753(e)(2): A multiple lift rigging assembly was not specifically designed with a maximum capacity for total assembly and for each individual attachment point. This capacity was not certified by a manufacturer or a qualified rigger with a 5 to 1 safety factor for all components. Location: Jobsite A worker was seriously injured when a piece of steel dropped where a multiple lift rigging assembly, which did not meet the requirements of 29 CFR 1926.753(e)(2) was used.

The employer did not ensure that each employee who performs multiple lift rigging has been provided training in the following areas: The proper procedures and equipment to perform multiple lifts required by � 1926.753(e). An employee, who was not specifically trained in multiple lift rigging procedures and their components, was injured when a piece of steel dropped from a multiple lift rig and struck the employee's arm.
