29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:       a)  On or about July 15, 2019, employees were exposed to eye injuries while using Metabo HPT Pneumatic Nail Guns while installing shingles in that they were not wearing safety glasses meeting the ANSI/ISEA Z87.1-2010, Occupational and Educational Personal Eye and Face Protection Devices.    b) On or about July 15, 2019, employees were exposed to eye injuries while using Metabo HPT Pneumatic Nail Guns while installing shingles in that they were using sun glasses not meeting the ANSI/ISEA Z87.1-2010, Occupational and Educational Personal Eye and Face Protection Devices.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On or about July 15, 2019, employees performing re-roofing job tasks on a single story residential building were exposed to a 7 foot fall hazard, in that, a Gorilla Ladders 18 foot Reach Multi-Position Ladder MPX-17 extended 11 feet was not extended 3 feet above the landing surface of the roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:   a) On or about July 15, 2019, employees conducting re-roofing activities using a Gorilla 18 foot Reach Multi -Position Ladder was not used at an adequate angle in that it was extended eleven feet and the distance between the foot of the ladder and the wall was four feet, seven and 5/8 inches.
