29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about July 15, 2019, the employer failed to ensure the employee would not free fall more than 6 feet. The employee was performing work on the roof and had not tightened up his lanyard to prevent him from free falling. Employee exposed to a fall hazard of approximately 9.5 feet to the lower level. In the alternative: 29 CFR 1926.104(d): Safety belt lanyard(s) worn by employee(s) were of a length that would provide for a fall greater than 6 feet:

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about July 15, 2019, the employer failed to ensure the employee was trained to recognize the hazard and safeprocedures in order to minimize the hazards. The employee was working on the roof and had not tightened up his lanyard to prevent him from free falling. He was not trained on how to wear his fall protection correctly.
