29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Production Room, Washer/ Dryer Area: The fan for a motor was not properly covered. Production Room, Bundling Area: The tie wrap machine was not properly designed to prevent the operator's fingers or hand from being struck by the descending plate. Production Room, Pressing area: The rotating mechanism of the shirt press machine was not adequately guarded.

29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335. Worksite: Training on electrical hazards was inadequate. Tools and parts were in the electrical control box for the washer dryer shuttle.
