29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:    Location:  Trench    Underground electric utilities which crossed the trench were unsupported.  The concrete encased electrical duct bank failed failed and caused an explosion which burned an employee.
