(a) On or about July 5, 2019, employee(s) were servicing Aaon Rooftop Air Conditioning Units (Model No. RN-015-3-0-EA09-14A) and did not ensure the air conditioning units were properly shut down and all power sources were isolated. This exposed employees(s) to rotating fan blades and amputation injury.

29 CFR 1910.28(b)(13)(iii)(B): 29 CFR 1910.28(b)(13)(iii)(B): The employer did not implement and enforce a work rule prohibiting employees from going within 15 feet (4.6 m) of the roof edge without using fall protection in accordance with paragraphs (b)(13)(i) and (b)(13)(ii): (a) On or about July 5, 2019, employee(s) servicing air conditioning units on the rooftop of an industrial building were exposed to a fall hazard of approximately 25 feet. A work rule covering building rooftop work and fall protection requirements, was not developed and implemented.
