29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about July 11, 2019, the employer failed to ensure employees performing soffit work from an elevated platform were protected from falls of approximately 13 feet in that no personal fall arrest systems/devices were in place.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about July 11, 2019, the employer failed to ensure employees were protected from a fall of approximately 16 feet while utilizing an 18 foot aluminum ladder to gain access to an upper landing area in that the ladder only extended approximately one (1) foot over the roof line.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   On or about July 11, 2019, the employer failed to ensure employees performing roofing work were protected from a fall of approximately 16 feet in that no fall protective systems/devices were utilized.   Weaver Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number1095754, citation number 1, item number 002 and was affirmed as a final order on October 13, 2015, with respect to a workplace located at 48 Medina Line Road, Sharon Center, OH 44274.
