29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) Worksite:  On or about 7/15/2019, employees were not protected from falling while working from a scaffold that was approximately twenty three feet above a lower level at the scaffold end-cap when receiving a tub of cement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Worksite - Mezzanine:  On or about 7/15/2019, the employer did not ensure that employees were protected from falling at a height of approximately seventeen feet above the lower level, when cable guardrails were taken down to receive concrete blocks and cement on the mezzanine level.    J.D. Cement Works, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1217504, citation number 001, item number 01 and was affirmed as a final order on 5/03/2017, with respect to a workplace located at 184 Woonasquatucket Avenue, North Providence RI, 02911.
