On or about October 31, 2019, at the Phillips 66 facility located in Ponca City, OK, the employer did not document that equipment complied with recognized and generally accepted good engineering practice such as ASME Boiler and Pressure Vessel Code Section VIII subsection UG-119 when it failed to ensure that the nameplate on Hot Oil Drum D-6603 was located in a conspicuous place on the vessel.
