29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about July 5, 2019, during commercial framing activities, the employer did not ensure employees were protected from fall hazards of approximately 20 feet by ensuring a fall protection system/device was being used.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about July 5, 2019 the employer did not report a hospitalization of an employee who fell approximately 20 feet and was hospitalized. The employee was performing carpentry work on an elevated surface with no fall protection on a commercial construction project.
