(a) Jobsite, McKnight Circle, Pittsburgh, PA, on or about July 16, 2019 employees were not protected from possible cave in hazards during trenching and excavation operations due to trench box being greater than 2 feet from the bottom of the trench.

Designs of support systems, shield systems, and other protective systems shall be selected and constructed by the employer or his designee and shall be in accordance with the requirements of paragraph (c)(1); or, in the alternative, paragraph (c)(2); or, in the alternative, paragraph (c)(3); or, i the alternative, paragraph (c)(4) as follows: (a) Jobsite, McKnight Circle, Pittsburgh, PA, on or about July 16, 2019 employees were not protected from soil and other materials from falling into the trench due to trench box not installed in accordance with manufacturers specifications.
