On or about July 17, 2019 and at times prior thereto, employees were replacing shingles on a one story residential, multifamily duplex building, at heights ranging from 8 feet to 12 feet above a lower level without the use of fall protection, exposing employees to the hazards of falling from heights 6 feet or more from a lower level.
