29 CFR 1910.27(b)(1)(i): Before any rope descent system was used, the building owner did not inform the employer, in writing that the building owner has identified, tested, certified, and maintained each anchorage so it is capable of supporting at least 5,000 pounds (268 kg), in any direction, for each employee attached. The information must be based on an annual inspection by a qualified person and certification of each anchorage by a qualified person, as necessary, and at least every 10 years:  (a)Gateway Square, LLC at 212 East 11th Street, Sioux Falls, SD 57104: On or about and at times prior to May 1, 2019, the creating and correcting employer did not ensure anchorages that were identified, tested, certified, and maintained so it was capable of supporting at least 5,000 pounds in any direction, for each employee attached. Subcontractor window washer employees wrapped ropes around an existing concrete structure located on the roof of the Avera building to use as an anchorage involving a rope descent system. The building owner has not evaluated the roof and identified a means of anchorage. This exposed subcontractor employees to a fall hazard of over 40 feet.
