29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about July 17, 2019, the employer exposed employees working on the ground or lower edges of the roof to struck-by hazards by not providing hardhats to protect their heads from falling or flying objects such as but not limited to clay shingles and tools that could cause physical harm.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    On or about July 17, 2019 and times prior thereto, employees performing roofing work, worked from inside a trash box elevated by a SS-842 Square Shooter Turbo forklift.  The trash box was used as a work platform that was not approved by the forklift manufacturer exposing the employees to an unprotected fall hazard

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about July 17, 2019, employees installing clay roofing materials on the roof were not provided fall protection and were exposed to unprotected fall hazards of up to approximately 14 feet to the concrete below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about July 17, 2019, employees performing roofing activities used a ladder to gain access on/off the roof and the ladder was not extended at least 3 feet above the upper landing surface exposing the employees to fall hazards.
