29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied. On or above July 17, 2019, an all-terrain boom forklift with an attached platform was used to lift employees was observed moving horizontally while the platform was occupied. This condition exposed employees to fall hazards greater than 10 feet.

29 CFR 1926.503(c)(3): 29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee. On or above July 17, 2019, employees have not been re-trained in fall protection hazards before riding a basket lifted by a all terrain boom lift. This condition exposed employees to fall hazards greater than 6 feet.
