29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:     On or about July 16, 2019, and at times prior thereto, containers containing Isopropyl Alcohol were being used near areas where open flames or radiation heat was present:         1. In the core molding area  an Isopropyl Alcohol container was found located approximately 18 feet from the areas where hot cores are  located.       2. In the pour area Isopropyl Alcohol container was found located approximately 10 feet from location where hot molten metal is poured into the molds.   Employees were exposed to fire hazards.

29 CFR 1910.106(e)(6)(ii): Class I flammable liquid(s) were dispensed into containers without electrically interconnecting the nozzle and the container:   On or about July 16, 2019, and at times prior thereto, in the core molding and pour area the employer allowed employees to dispense Isopropyl Alcohol into another container without  effectively grounding and bonding to electrically interconnect the containers exposing employees to a fire hazard.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   On or about July 16, 2019, and at times prior thereto, at the facility the employer allowed employees to pour hot metal molten into molds without wearing heat resistant long sleeves.  The employees were exposed to burn hazards.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    On or about July 16, 2019, at the establishment CSHO observed several forklift truck operators whose certification  was expired by more than a year.  This exposed employees to struck-by  hazards.
