29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not be protected by a personal fall arrest system.  A.           On or about 07/18/19, in the back of the house of 246-16 60th Ave, Little Neck, NY    Employees using a ladder jack scaffold were not protected from falling approximately 13 ft. to the ground.   ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Worksite: Employees installing eave for new two-story house  on a ladder jack scaffold approximately 13 ft. above ground  were not using any fall protection.  The employees who performed work while on the scaffold did not recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. on or about 7/18/19.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
