29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:      The employer is failing to protect employees from cave-in crushing hazards. This was most recently documented at a worksite located at or near 2909 Marconnit Circle, Norfolk, Nebraska 68701 on or about July 18, 2019. The employer is failing to ensure that a spoil pile remained at least two (2) feet away from the edge of the trench/excavation while utility work was being performed.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal):              The employer is failing to protect employees from cave-in crushing hazards. This was most recently documented at a worksite located at or near 2909 Marconnit Circle, Norfolk, Nebraska 68701 on or about July 18, 2019. The employer is failing to ensure that employees were adequately protected from cave-ins while in the process of doing plumbing work when employees were in the excavation.

This was most recently documented at a worksite located at or near 2909 Marconnit Circle, Norfolk, Nebraska 68701 on or about July 18, 2019. The portable ladder being used by employees to access the excavation did not extend at least 3 feet above the surface of the excavation.
