29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:    Daniel Vazquez worksite located 2235 King St., Toledo Ohio 43615:  On or about July 18, 2019, the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.

Daniel Vazquez worksite located 2235 King St., Toledo Ohio 43615:  On or about July 18, 2019, the employer did not ensure employees were protected from eye injuries while replacing sheathing on single story residential structure using pneumatic nailers.

Daniel Vazquez worksite located 2235 King St., Toledo Ohio 43615:  On or about July 18, 2019, the employer did not ensure employees were protected from falls while tearing off shingles and making roof repairs on the single story residential structure. The employees were exposed to a fall distance of approximately 8 feet from a steep slope roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  Daniel Vazquez worksite located 2235 King St., Toledo Ohio 43615:  On or about July 18, 2019, the employer did not ensure the extension ladders was being used at an angle to prevent displacement. An employee using the ladder to remove shingles from a single story roof in this condition was exposed to falls of approximately 8 ft.
