29 CFR 1926.97(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum intervals between tests were not in accordance with Table E-4 and Table E-5.  In the alternative,  29 CFR 1926.97(c)(2)(xii): The employer did not certify that the equipment has been tested in accordance with the requirements of paragraphs (c)(2)(viii) of this section. The certification shall identify the equipment that passed the test and the date it was tested and shall be made available upon request.  Site: 7620 15th Avenue, Brooklyn, NY 11228  Location: a) On the South-East roof of the 1st floor.  a) The employer provided employees with a pair of Marigold Industrial insulating rubber gloves that were not tested, and labeled, within the past 12 months prior to first service issue date. The condition was noted at or about 07/16/2019.

Site: 7620 15th Avenue, Brooklyn, NY 11228  Location: a) Towards the South-East, 15th Avenue, entrance on the roof of the 1st floor to the Driveway floor  On or about 07/16/2019, a power strip was used outdoors for a purpose that it was not intended for in construction. Employees energized the power strip by using an orange electrical cord which was tapped into the main line service drop. This power strip was used to energize a hand-held power drive pipe-threading machine and battery charger packs.

On or about 07/16/2019, an employee was working on the roof of the 1st floor and exposed to falling approximately 12 feet to the next lower level while using an extension ladder for access to and from the roof. The ladder was placed on the driveway exterior wall of the house, which was not the landing level that employees accessed.
