29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equaled or exceeded an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: (a) In the manufacturing building, a chop saw operator was exposed to noise levels at 328.0% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 328.0% is approximately 98.5 dBA. Sampling was performed for 471 minutes during one shift. Zero exposure was assumed for the unsampled period of 9 minutes. (b) In the manufacturing building, a chop saw operator was exposed to noise levels at 167.3% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 167.3% is approximately 93.7 dBA. Sampling was performed for 472 minutes during one shift. Zero exposure was assumed for the unsampled period of 8 minutes. (c) In the manufacturing building, a trim saw operator was exposed to noise levels at 189.8% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 189.8% is approximately 94.6 dBA. Sampling was performed for 470 minutes during one shift. Zero exposure was assumed for the unsampled period of 10 minutes. (d) In the manufacturing shed, a trim saw operator was exposed to noise levels at 72.53% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 72.53% is approximately 87.6 dBA. Sampling was performed for 467 minutes during one shift. Zero exposure was assumed for the unsampled period of 13 minutes. (e) In the manufacturing building, a pallet dismantler operator was exposed to noise levels at 318.3% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 318.3% is approximately 98.3 dBA. Sampling was performed for 473 minutes during one shift. Zero exposure was assumed for the unsampled period of 7 minutes. (f) In the manufacturing building, a pallet dismantler operator was exposed to noise levels at 471.9% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 471.9% is approximately 101.1 dBA. Sampling was performed for 463 minutes during one shift. Zero exposure was assumed for the unsampled period of 17 minutes. (g) In the manufacturing building, a pallet repair operator was exposed to noise levels at 199.2% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 199.2% is approximately 94.9 dBA. Sampling was performed for 476 minutes during one shift. Zero exposure was assumed for the unsampled period of 4 minutes. (h) In the manufacturing building, a pallet repair operator was exposed to noise levels at 178.2% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 178.2% is approximately 94.1 dBA. Sampling was performed for 474 minutes during one shift. Zero exposure was assumed for the unsampled period of 6 minutes. (i) In the manufacturing building, a pallet repair operator was exposed to noise levels at 128.4% of the permissible 8-hour TWA (90 dBA). The equivalent dBA level of 128.4% is approximately 91.7 dBA. Sampling was performed for 463 minutes during one shift. Zero exposure was assumed for the unsampled period of 17 minutes.
