29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Worksite, 2282 Bellmore Ave, Bellmore, New York - An employee was using a NITTO KOHKI Portable Automatic Drilling Machine that had multiple cracks and tears along the jacket of the flexible power cord, exposing live wires and no visual inspection was conducted; on or about 04/18/2019. b) Worksite, 2282 Bellmore Ave, Bellmore, New York - An employee was using a yellow flexible power cord to power a NITTO KOHKI Portable Automatic Drilling Machine with a damaged jacket, exposing live wires and a visual inspection was not conducted; on or about 04/18/2019. c) Worksite, 2282 Bellmore Ave, Bellmore, New York - An employee was using an MILLER MILLERMATIC 350P MIG Welder with a damaged 220 Volt connector and a visual inspection was not conducted before the work shift; on or about 04/18/2019. d) Worksite, 2282 Bellmore Ave, Bellmore, New York - An employee was using an orange flexible power cord with a damaged jacket, exposing live wires and a visual inspection was not conducted; on or about 04/18/2019.
