29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on energized circuits and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools: a) Facility, 23rd Street & Duss Avenue, Ambridge, PA: On or about July 17, 2019, employees were exposed to electrical shock/electrocution hazards in that circuits were not initially de-energized to perform a visual inspection of the ICO cabinets hazards and proper precautionary methods and personal protective equipment were not utilized. Employees tested ungrounded conductors without personal protective equipment.
