On 7/18/2019, Employees of Ace Builders, Inc. were constructing a commercial building located at 3090 Elm Point Industrial Drive, St. Charles, MO.  The employees were working from an JLG 460SJ aerial lift, Serial number 354445 which had the boom in the elevated position approximately twenty four feet above the ground.  The employees were inside the boom working without the use of body belts or body harnesses and lanyards not attached to the boom or basket of the aerial lift.

The V.P. and four employees were performing work on the leading edge of the roof, which was approximately 24 feet above the ground, without the proper use of guardrail systems, safety net systems, or personal fall arrest systems or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      Ace Builders, Inc. was previously citied for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1072392,  citation number 1, item number 1 and was affirmed as a final order on 7/9/2015, with respect to a workplace located at 8900 East Broad Street ,Pataskala, OH 43062.
