On or about July 22, 2019, throughout the facility an employee operated a UniCarriers 80 brand forklift, model MJ1F4A35LV SN J1F4-981785, with the seatbelt buckled behind the seat.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about July 22, 2019, in the machine area, located in the north east corner of the building, where employees were exposed to caught-by hazards while operating a manual Chinhung brand CH-430 metal lathe (MS 1040) without a guard to cover the rotating chuck.

In the machine shop area, located in the northeast corner the building, where employees were exposed to caught-by and struck-by hazards while using a Dahlih brand mill (MS1050) without a point of operation guard.  b. In the production shop area, located on the west wall of the building, where employees were exposed to caught-by and struck-by hazards while using a Continental Machine Co. roll saw (C1020, SN 2395) without a point of operation guard.
