PS Furniture, Inc. - On or about 7/22/19, and at times prior thereto, employees were performing servicing and maintenance on equipment/machinery that included, but was not limited to, the thermoform machine, the robot, and the tubing bender. The lockout/tagout procedures did not list the magnitudes of energy, specific procedure steps for shutting down the equipment, or how to test the equipment to verify proper de-energization.
