29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) Site: In the Compounding Area, Building A, Lower Level, Lines 1-3 and Building B, Lower Level, Lines 4-5 in the production area at the facility located at 170 Pioneer Drive, Leominster, MA 01453: On 11/8/2019, and at times earlier, energy control procedure Document No. QLWI-7.5.1.5-3, Rev 00 and entitled Lock-Out Tag-Out (LOTO) Machine Specific Procedure Dicers for the Bolton Emerson Dicer did not properly identify the scope (i.e. Dicers 1-5), it did not have clear and specific steps to verify pneumatic energy isolation and release, it did not include specific steps for the proper removal of LOTO devices, nor did it include means to enforce compliance.
