29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: a) For the employees cleaning the Urschel cutters on or about July 22, 2019 and times prior thereto the employer did not develop and implement procedures to verify equipment was de-energized prior to employees removing guards or covers to access the point of operation of the equipment. b) For the employees replacing the cutter head blades on the Urschel cutters on or about July 22, 2019 and times prior thereto the employer did not develop and implement procedures to verify equipment was de-energized prior to employees removing guards or covers to access the point of operation of the equipment.
