29 CFR 1926.451(g)(4)(i): Guardrail systems was not installed along all open sides and ends of platforms. Guardrail systems was not installed before the scaffold is released for use by employees other than erection/dismantling crews. On or about June 5, 2019. Employees were exposed to a fall hazard of approximately 13 feet in height when working on a 3 tier scaffold without guard rails on the open ends.
