29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) New residential construction located on Lot #48, Front side: On or about 07/17/2019, and at times prior, the employer exposed employees to fall hazards of 15-feet, 8-inches to the lower level, in that a fall protection system was not provided to an employee installing siding from a pump jack scaffold system.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: (a) New residential construction located on Lot #48, Front side: On or about 07/17/2019, and at times prior, the employer exposed employees to fall hazards of 15-feet, 8-inches to the lower level, in that a ladder section was used as a plank which was strapped to the aluminum walk board to make the scaffold longer. The ladder was not designed to be used as a plank or work platform.
